We are participating providers for Medicare and most major insurances. Please check with your insurer to find out if we are preferred providers. This usually determines your out-of pocket cost. We will submit claims to your carrier in your behalf. We bill insurance companies, including Medicare as PMK Medical Group, Inc.
Please inform our office if pre-certification is necessary with your plan. Some insurance companies require you to use a specific laboratory, imaging center or need pre-certification for office visits, hospital admissions and treatment. It is your responsibility to obtain the initial referral on your first visit.

Regarding Infusion Treatments:
Billing statements for infusion treatments will come from Dignity Health St. John's Regional Medical Center. If you have any questions about your infusion treatment bill, please call Patient Financial Services at (877) 877-8345.
If you are not 100% covered by insurance, please call Dignity Health Patient Financial Advocate, Annette at (805) 988-2500 ext. 3983, about possible financial assistance programs offered by the manufacturer and foundations before you start infusion treatment.
